Waiver of Liability and Informed Consent Release

 

I have enrolled myself and my dog(s) in dog training classes, private consultations, workshops or other related activities (Classes) offered by Andrea Acosta and Spot On Training, LLC (collectively, “Spot On”).

By signing below, I acknowledge that I have been informed, understand and accept that there are inherent and unavoidable risks associated with training, socializing, walking and working with dogs in general, and especially with dogs that have behavioral issues. Additionally, I have had full opportunity to discuss all concerns I have about the foregoing risks with Spot On and its authorized representatives. I hereby accept and assume, without reservation, all risks associated with my participation in the Classes, including, but not limited to: the risks of any and all injuries to myself, my dog and any of my family members or third parties who may attend; the risks that my dog may cause injury to other persons and/or dogs while training. I further understand that due to the way that dogs interact with one another, cuts or scratches can occur to me or my dog, even though the dogs are carefully supervised. I also understand that my participation in any of these activities is not without risks such as tripping, falling, getting knocked down and jumped on, bitten, scratched, etc.

I also understand that in enrolling my dog(s) into any of Classes, that Spot On has relied upon my representation that my dog(s) are in good health and have received the necessary vaccinations (or equivalent alternatives) as agreed upon by my veterinarian. If my dog becomes injured or ill, Spot On shall have the right but not the obligation to call any veterinarian that I may designate to Spot On from time to time. In the event of an emergency and my designated veterinarian is unavailable, Spot On has the right to take my dog(s) to the nearest animal hospital for immediate care. I agree to reimburse, in full, Spot On for any medical expenses accrued while under the supervision of Spot On. Spot On will not be held responsible for any decisions made by the veterinarian on the dog’s behalf.

I hereby release and agree to indemnify and hold harmless, Spot On, Andrea Acosta, and any affiliates or property owners from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I understand that I am solely financially responsible for any damage or harm caused by my dog during training, boarding, socializing, or walking.

I take it upon myself to ask questions and use my own common sense. I understand that I do not have to do anything or allow anything to be done that I do not feel comfortable with.

I certify that I have read and understand the rules and regulations set forth herein and that I have read and understand this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions and statements of this agreement and confirm the truthfulness of the contents of this release form completed by me.
